As a registered member of Eastern Pennsylvania Soccer, the following insurance programs are available at no cost.
Participant Accident Insurance NEW PROCEDURE (JANUARY 2024)
This is secondary to a player’s primary insurance, or will apply in the event that a player is not currently insured. This covers you during any sanctioned practice, game or tournament.
In order to file a claim if you are injured during a sanctioned EPSA event:
1) All player accident claims must be submitted through our new online insurance claim process. Paper claims will no longer be accepted. You are directed to submit your claim directly to USASA on the site below :

2) You should make a copy of the one page submission for your records and send a copy to the EPSA Office, as well as
1. Contact your League Representative
2. You’ll need to secure a copy of the referee game report
3. Gather medical bills and records, as you may need them later
4. Complete the claim form, making sure all pertinent information is filled in, including the appropriate signatures.
5. Mail a copy of the insurance claim form and referee report to:
EPSA Administration, 4070 Butler Pike, Suite 100, Plymouth Meeting PA 19462.
Once received and approved by USASA and the Insurance Company for final approval. You will be notified in writing by email from cdxnotify@agadministrators.com and affinity@agadm.com
Facility Insurance NEW PROCEDURE ( August, 2022)
All fields and facilities, used by affiliated teams and leagues, are eligible for liability insurance through USASA.
A team needs a Liability Insurance Declaration Certification (COI) for any field not owned by that team or its respective team club. Any team intending to play EPSA sanctioned match(es) or competition(s) at a site that is not owned by that team or its respective team club MUST submit a request form from EPSA for a Liability Insurance Declaration Certification (COI) with a copy of a contract/field usage permit ( FUP) from the individual site to be utilized in advance of any match to be played at that respective site, identifying the date(s), time(s), and physical location of that site. Then EPSA will issue a date stamped and authorized Liability Insurance Declaration Certification to the team manager/coach for that specific site. The Liability Insurance Declaration Certification will only be applicable to one specific site and the times and dates submitted on the contract/field usage permit. This does not waive or approve the field qualification requirements established in the EPSA Cup policies.
Complete this form and return it along with the signed field contract/field usage permit (FUP) to the State Office: EPSA, 4070 Butler Pike, Suite 100, Plymouth Meeting PA 19462.. Upon receipt and approval, the COI will be issued within 1-3 days and emailed back to the requester.
